Join our brand-new team of Nurse Coordinators
At Toronto Central Community Care Access Centre (CCAC), we continually focus on new ways to deliver excellence and help clients and their families get the health care services and support they need. We are implementingn several innovative new programs and are looking for a number of RNs who, as Nurse Coordinators, will play a key, hands-on role in meeting the needs of Torontonians. These are exceptional opportunities for RNs who wish to explore case management while maintaining and utilizing their hands-on clinical skills in a different kind of health care environment, to join one of the following brand new teams:
School Mental Health & Addictions Nursing Team - An RN with 2+ years of strong clinical experience and knowledge of the school mental health and addictions service system for children and youth, you're ready to get in on the ground floor of this exciting program. Along with providing health-related advice and strategies for parents, teachers and schools to build capacity in recognizing and addressing students' issues and needs in this critical area, you will help students and parents access services such as family health care, community mental health and/or addictions agencies. English/French bilingualism is required for some positions.
Telehomecare Nursing Team - Using your 2+ years of clinical and community health experience as an RN and your knowledge of chronic disease management, you will handle an ongoing caseload by providing remote client monitoring, health coaching and disease-specific education to individuals with complex chronic diseases. Your goal will be to encourage self-management by working collaboratively within the client's circle of care and connecting him or her with other community resources.
Rapid Response Nursing Team - The first 24 hours following a patient's discharge from hospital can often be filled with anxiety and uncertainty. That's where you come in. As a Rapid Response Nurse Coordinator, you will apply your 3 to 5 years of relevant clinical experience as an RN to ensure effective transitions from acute care to home care for target populations, such as medically complex children and frail adults, and seniors with complex needs and/or high-risk characteristics such as congestive heart failure.
Registration as an RN in good standing with the CNO is essential for all roles, as is the ability to work with culturally diverse groups. Full position details are available online at ccacjobs.ca
